This Privacy Policy explains how Vital View 3D Breast Imaging (“Vital View 3D,” “we,” “us,” or “our”) collects, uses, shares, and protects personal information when you visit vitalview3d.com (the “Site”), contact us, request an appointment, or receive communications from us. It also explains the choices and rights you have.
Health information. Because we are a breast imaging center, some information you share with us (for example, why you are requesting a scan) is health-related. Our separate Consumer Health Data Privacy Policy, required by Washington’s My Health My Data Act, explains how we handle that information. Where the two policies differ, the Consumer Health Data Privacy Policy controls for consumer health data.
1. Information we collect
Information you give us. When you fill in a form on the Site, email, call, text, or fax us, or visit our center, you may provide: your name, email address, phone number, and mailing address; the services you are interested in; details about your appointment request, including health-related information such as symptoms, prior imaging, breast density, or family history; the name and contact details of your referring provider and the imaging order they issue; messages you send us; and, if you opt in, consent to receive text messages.
Information collected automatically. When you use the Site, our hosting provider and analytics tools automatically collect technical information such as your IP address, browser and device type, pages viewed, the site that referred you, and the date and time of your visit. We use Google Analytics to understand how the Site is used, Google reCAPTCHA to protect our forms from spam, and an embedded Google Map on our Contact page. Each of these is provided by Google and is subject to the Google Privacy Policy. Google may set cookies and collect information about your device and usage when these features load.
Information from other sources. We may receive information from your referring provider (such as an imaging order) and from social media platforms when you interact with our pages there.
2. How we use information
- To respond to your enquiry, schedule and confirm appointments, and provide breast imaging services.
- To communicate with you and your referring provider about your care, results, and follow-up.
- To send appointment reminders and, where you have opted in, educational or marketing emails or text messages. You can opt out at any time.
- To operate, secure, and improve the Site, including preventing spam and abuse.
- To comply with legal obligations and protect our rights.
3. Cookies and analytics
The Site uses cookies and similar technologies for essential functions, security (reCAPTCHA), and analytics (Google Analytics). You can control cookies through your browser settings; disabling cookies may affect some features. You can opt out of Google Analytics using the Google Analytics opt-out browser add-on. We do not currently respond to browser “Do Not Track” signals.
4. Text messaging (SMS)
If you provide your mobile number and opt in, we may send you appointment-related and service messages by text. Message frequency varies; message and data rates may apply. Reply STOP to unsubscribe or HELP for help. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information we share with service providers is used only to help us send messages to you. Our SMS Terms and Conditions apply to our text-messaging program.
5. How we share information
We do not sell your personal information, and we do not share it with third parties for their own marketing. We share information only:
- With service providers who help us run the Site and our business and who are permitted to use it only on our behalf: website hosting (GoDaddy), form processing (WPForms), email delivery (Brevo), business email and documents (Microsoft 365), analytics, spam protection, and maps (Google), and our imaging, scheduling, and payment systems.
- With your health care providers, such as the provider who ordered your scan, to coordinate your care.
- When required by law, such as to respond to a lawful request, protect the safety of any person, or enforce our terms.
- In a business transfer, if we are involved in a merger, sale, or reorganization, subject to this Privacy Policy.
6. How long we keep information
We keep personal information for as long as needed for the purposes above, and health care records for the periods required by Washington law. Website form submissions and analytics data are kept only as long as reasonably necessary for the purposes described in this policy.
7. Security
We use reasonable administrative, technical, and physical safeguards to protect personal information, including encrypted connections (HTTPS), access controls, and spam protection on our forms. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Please do not send detailed medical history through the website contact forms; we will collect what we need at your appointment.
8. Your choices and rights
- Marketing. Unsubscribe from emails using the link in any message, or reply STOP to any text message.
- Access, correction, and deletion. You may ask us to confirm what personal information we hold about you, correct it, or delete it, subject to legal record-keeping requirements. Washington residents have additional rights over consumer health data, described in our Consumer Health Data Privacy Policy.
- Health care records. Requests to access or amend your health care records are handled under Washington’s Uniform Health Care Information Act (RCW 70.02) and, where applicable, HIPAA.
To exercise any of these rights, contact us using the details below. We will verify your identity before acting on a request and will not discriminate against you for exercising your rights.
9. Children
The Site is intended for adults. We do not knowingly collect personal information from children under 13 through the Site. If you believe a child has provided us information, contact us and we will delete it.
10. Third-party links
The Site links to other websites, such as news coverage and Google Maps. Those sites have their own privacy practices, which we do not control.
